Medical Appointment Transport
The Colac Area Health Transport Program provides door-to-door transportation for current Colac Area Health clients and eligible CHSP community members who need assistance getting to and from medical appointments.
Who is Eligible?
This service is available to:
- Community members eligible under the Commonwealth Home Support Programme (CHSP)
- Current Colac Area Health clients who require transportation assistance under 65 years old
All eligible individuals must:
- Have a valid medical appointment
- Live within our service area
- Have limited access to other transportation options
How Our Service Works
Step 1: Register for Transport
Register with My Aged Care (over 65yo or over 50yo if identify as Aboriginal)
Step 2 Request medical appointment transport
Request transport at least one week in advance of your appointment

Step 3: Processing Your Request
Our Volunteer Coordinator reviews your request and works to match you with an available volunteer driver and vehicle.
Step 4: Confirmation
You will receive confirmation once your transport has been arranged.
Step 5: Day of Service
Your volunteer driver will arrive at the scheduled time, transport you to your appointment, and return you home afterward.
How to Register for Transport on My Aged Care
If you are over 65 or identify as Aboriginal and Torres Strait Islander over 50yo – register with My Aged Care for Transport and we will get your referral and be in touch.
